Adams explained the MIH team is called upon after a physician makes a referral, normally following an ER visit. Once a logistics officer ensures eligibility, the team arranges a home visit.

The program is in its fourth year, operates across three Michigan counties and is designed to drive down costs associated with costly ER visits, especially for patients who likely require care that is not an immediate emergency.
